ORDER CORRECTING ERROR IN DECISION 09-07-019

The Commission has been informed of an error in Decision (D.) 09-07-019. The form for reporting General Order (GO) 133-C Service Quality Standards Reporting, attached to Attachment 1 of the proposed decision, inadvertently was omitted from D.09-07-019. This decision confirms that the reporting form, attached to this decision, is a part of GO 133-C and the end of Attachment 1 to D.09-07-019. Therefore, pursuant to Resolution A-4661 and Rule 16.5 of the Commission's Rules of Practice and Procedure, which provide for the correction of obvious omissions in a Commission decision,

IT IS ORDERED that Decision 09-07-019 is corrected as follows:

1. The General Order 133-C Service Quality Standards Reporting form, attached to this decision, is part of General Order 133-C and the end of Attachment 1 to Decision 09-07-019.

2. Rulemaking 02-12-004 is closed.

This order is effective today.
