List of deficiencies in the tariff filed by Metropolitan Telecommunications of California, Inc., d/b/a MetTel in A. 01-06-010 to be corrected in its tariff compliance filing.

1. Add the words "Schedule Cal. P.U.C. No 1-T" on the top right-hand corner of every tariff sheet.

2. Remove both dates on the bottom right-hand corner of every tariff sheet. Indicate the date filed during the initial tariff filing, and the Telecom Division will date stamp the effective date.

3. Sheet 6: The tariff must be available for inspection at an office in California.

4. Sheet 130, Rule 11: State that Basic Service will not be disconnected for non-payment of anything other than residential and single line business, Flat Rate and/or Measured Rate Service as defined in D. 96-10-066, Appendix B, page 5. Refer to D. 00-03-020.

5. Sheet 134, Rule 14: Remove Rule 14 from the tariff and include Pacific or Verizon's Limitation of Liability tariffs regarding credit for service interruptions. Refer to D.95-12-057.

6. Sheet 140, Rule 17: Remove Rule 17 from the tariff and include Pacific or Verizon's Limitation of Liability tariffs. Refer to D.95-12-057.

7. Sheet 152, Rule 21: Include the revised surcharges shown in Conclusion of Law 4.

8. Sheet 154 and 155: Include the following Income Limitation and charge for ULTS:
